Reentry Peer Connection Program
Lived Experience. Trusted Support. A New Phase Forward.
A community-based, peer-led program supporting individuals returning home after incarceration, detention, or other justice-system involvement. Participants are matched with trained peer support specialists who have relevant lived experience and offer mentoring, individualized reentry planning, resource navigation, advocacy, and steady encouragement. Support can begin before release and continues throughout the transition back into the community — because a person's past does not eliminate their potential.
